Hidden Acne (Closed Comedones): How Long Treatment Takes

How long do closed comedones take to clear? Usually 8–12 weeks with the right plan. Free dermatologist consult in HCMC; hidden-acne program 1,100,000 VND.

Medically reviewed by Dr Quynh Trang, Dermatologist (HCMC) · Updated August 1, 2026

Closed comedones — the bumps most people call hidden acne — usually take 8–12 weeks to clear on a properly built plan, with the first noticeable change at 2–4 weeks. The timeline is set by biology rather than by effort: the plug sits under the horny layer with no way out, so it can only surface as the skin turns over, roughly once every 28 days. A course therefore runs across two or three of those cycles. Very oily skin, a thick horny layer or areas that have been squeezed repeatedly all take longer. At WeDo (District 1 and District 2, Ho Chi Minh City), the consultation and multi-layer skin analysis with a board-certified dermatologist are free; medical extraction starts at 390,000 VND per session and the hidden-acne program (MA2) is 1,100,000 VND per session.

Closed comedones cluster on the T-zone and chin, but this page does not walk through acne by facial region — for the full picture of acne types and the treatment ladder that covers them, start from WeDo’s medical acne treatment overview.

The technology

How it works

Medical acne extraction

Medical acne extraction

The follicle opening is cleared with sterile instruments and the plug is lifted out along the axis of the follicle rather than squeezed from both sides, so the follicle wall stays intact and nothing is driven back into the dermis. Indicated for closed comedones, whiteheads and fine congestion that have risen to a safe depth; anything still deep or already inflamed is left for a later visit. Repeated every few weeks through the course, with the number of sessions set after the skin analysis. Expect mild redness for a few hours up to a day, with no restriction on normal activity. Cost: 390,000 VND per session for the basic extraction course.

Medical peel to soften keratin and clear the follicle

Medical peel to soften keratin and clear the follicle

The dermatologist applies an agent that weakens the bonds between the keratinocytes packed at the follicle opening; the horny layer sheds more evenly, drainage returns, and comedones surface more readily at the next visits. Peel type, layer count and area are matched to keratin thickness, oiliness and sensitivity — there is no single formula. Indicated for thickened, congested skin, widespread fine comedones and oily skin with enlarged pores; postponed on actively inflamed or broken skin. Given as a course, several weeks apart, in step with the renewal cycle. Light flaking for a few days, and strict sun protection is mandatory throughout. Cost: 790,000 VND by area and 1,500,000 VND full face for renewing peels; 890,000 VND and 1,100,000 VND for the Dermalogica peels.

Hydration electrophoresis after extraction

Hydration electrophoresis after extraction

A very low-intensity current carries actives through the horny layer more effectively than topical application, concentrating on rehydration and calming the area just extracted. The goal is to keep the barrier strong enough that the skin does not answer with rebound oil production, since tight, dehydrated skin after a procedure is one of the reasons follicles seal again quickly. Indicated at most visits that include extraction, and particularly after a peel. Performed within the same session, with no downtime. This is an in-session step rather than a standalone line on the price list; its cost sits inside the program your dermatologist prescribes.

Celluma LED — the calming step after extraction

Celluma LED — the calming step after extraction

Low-energy red and near-infrared light is absorbed by skin tissue and used in the repair process, calming the treated area and reducing redness after an extraction visit. It creates no wound and removes no surface tissue. Indicated as the closing step of a session, or between sessions when skin needs extra recovery; the effect is cumulative, so keeping to schedule matters more than any single session. No downtime — makeup can go back on the same day. Cost: 330,000 VND per session.

The MA line — programs built for non-inflamed acne

The MA line — programs built for non-inflamed acne

MA is our internal designation for the programs built around hidden and non-inflamed acne; the number after it is a program code used for booking and billing, not a severity score. Each session is a complete sequence: cleansing, keratin softening, extraction, recovery and hydration. Indicated by degree of keratin build-up, oiliness and affected area, with the line and the number of sessions chosen after the skin analysis rather than sold as a fixed package. Sessions sit a few weeks apart, in step with the skin's renewal cycle. No meaningful downtime beyond mild same-day redness. Cost: 760,000 VND for MA1, 970,000 VND for MA1 Pro, 1,100,000 VND for MA2 — the hidden-acne program — 1,350,000 VND for MA1 Promax and 1,400,000 VND for MA3 where the skin needs intensive recovery.

Advantages

Why choose this treatment

A board-certified dermatologist reads your skin under multi-layer imaging to establish how deep the comedones sit before anything is prescribed.
Extraction is performed at the right moment, under sterile conditions, never by prying at lesions that have not gathered.
Plans are built around your keratin build-up and oil levels rather than sold as a fixed package.
Consultation, skin analysis and follow-up visits are free for the whole course.
Every line item is published, and the quote you receive matches it.
Regular reassessment so sessions that are no longer needed get dropped instead of run to the end of a package.

Treatment process

5 steps, clinically led

1

Step 1: Skin analysis and multi-layer imaging — free

Imaging shows what the eye misses: how deep the comedones sit, how thick the horny layer is, where the skin produces the most oil, and whether any inflammation is starting. This step is free, including if you decide not to treat.

2

Step 2: Your dermatologist reads the results and builds the plan

You are shown what is driving the congestion on your own skin, then given a plan matched to that mechanism — which steps, in which order, roughly how many sessions, and an itemized quote before you agree to anything.

3

Step 3: Soften the horny layer and reopen the follicle

The skin is deep-cleansed, then a medical peel or an appropriate keratin-softening step is applied so the horny layer sheds evenly and the follicle drains again — the precondition for safe extraction.

4

Step 4: Medical extraction and same-session recovery

Comedones that have reached a safe depth are removed with sterile instruments along the follicle axis; lesions that are not ready are left for the next visit. The skin is then hydrated, calmed and barrier-repaired in the same session so it does not rebound with excess oil.

5

Step 5: Follow-up on the skin's cycle, plan adjustment, then maintenance

Standardized before-and-after photos are compared, response is assessed area by area, and intensity or interval is adjusted. Once texture has settled, the course moves into maintenance with progressively wider spacing.

Aftercare

Make results last

Cleanse gently twice a day — and cleansing does not mean scrubbing. Congested skin invites rough handling because the texture makes people want to buff it away; that thins the barrier and drives sebum production up, which is precisely what you are trying to control.

Moisturize properly, oily skin included. Dehydrated skin compensates with more oil, and that oil feeds the same keratin plug you are treating. Choose non-comedogenic products and actually read the label: one occlusive moisturizer or sunscreen can extend a course on its own.

Sun protection after a peel is mandatory, not optional. A freshly thinned horny layer pigments faster than usual, and any mark that appears during this phase takes additional weeks to fade.

Do not squeeze at home, and skip comedone extractors and pore strips. Those tools remove what sits at the surface, leave the deeper portion behind and stretch the follicle opening in the process — trading a moment of clean skin for a longer-running problem.

Keep the next appointment. A comedonal plan works cumulatively across the skin’s renewal cycle, so moving visits around distorts the whole course rather than delaying it by one session.

Come back earlier than scheduled if a bump turns red, becomes tender, or an area starts spreading redness quickly — that means the lesion has turned inflamed and the plan needs reassessing. You can book a free consultation or follow-up at any point during your course.

FAQ

Still have questions?

How long does it take to get rid of closed comedones?
On a properly built plan, most people feel the skin become less bumpy within 2–4 weeks and need roughly 8–12 weeks for texture to settle, because comedones can only surface as the skin renews itself over about 28 days. Very oily skin, a thick horny layer or areas that have been squeezed repeatedly take longer, and your dermatologist reassesses at every follow-up.
Do closed comedones go away on their own?
Some small comedones do clear when the skin sheds on schedule, but the keratin plug sealing the follicle tends to persist for a long time and to spread into rough patches. Left alone long enough, a sealed follicle can become inflamed — at which point treatment takes more time and costs more.
Should you pop closed comedones?
No, not at home. A closed comedone has no opening yet, so outside pressure tends to rupture the follicle wall and push part of the plug deeper, producing inflammation and dark marks. In clinic, extraction happens only after the horny layer has been softened and the comedone has risen, using sterile instruments.
What is the difference between closed comedones and whiteheads?
Both are plugs sealed inside a hair follicle; the difference is depth. A closed comedone sits deeper, so you feel roughness rather than see a spot, while a whitehead has risen close enough to the surface to show as a pale dot. Treatment direction is the same — only the timing of extraction differs.
Why am I breaking out more after starting acne treatment?
Those are usually comedones that had already formed under the skin, surfacing once the horny layer is cleared and the follicles drain. The phase is typically short and eases over subsequent sessions; changing plans midway through it is one of the most common reasons a course ends up running long.
Can a chemical peel clear closed comedones?
A medical peel is one part of the answer rather than the whole of it: it weakens the bonds between keratinocytes at the follicle opening so the pore drains and comedones can rise, which is what makes safe extraction possible. Used alone it improves texture but leaves the plugs in place, so peels are combined with extraction and barrier recovery in a course.
How much does hidden acne treatment cost in Ho Chi Minh City?
At WeDo, the consultation and multi-layer skin analysis with a board-certified dermatologist are free; basic medical extraction is 390,000 VND per session, the hidden-acne program (MA2) is 1,100,000 VND per session, and the oily/acne-prone skin programs run from 760,000 VND to 1,400,000 VND per session. Your total depends on how many sessions your dermatologist prescribes after the skin analysis.
