What Is Cystic Acne, and Why Does It Form?

Every breakout starts the same way: a follicle blocked by oil and dead skin cells. Sealed off, it becomes a low-oxygen pocket where C. acnes multiplies fast, and your immune system floods it with white cells.

What happens next depends on the follicle wall. If it holds, the reaction stays shallow and you get an ordinary pustule. If it ruptures into the dermis, oil, keratin and bacteria spill into living tissue and the immune response scales up to match — the firm, red, tender lesion you can feel. Because that inflammation sits deep, it cannot come to a head quickly, and while you wait the collagen scaffolding around the follicle is being dismantled. When There Is No Visible Head

Nodular vs. Cystic Acne

The two words get used interchangeably, but clinically they differ in depth, and therefore in consequence. A cystic lesion is firm and well defined. Location does not diagnose the cause, but it narrows the list.
